If you are applying to the University Scholars Program, you must completea University Application that can be obtained by calling 330/941-2000 orat 1-800-336-9978. If you meet the requirements of the UniversityScholarsProgram and are not awarded one of the forty scholarship, you will qualifyto become a member of the Honors Program. Although there isn't ascholarship attached to the program, students chose to participate becausethey want to take challenging courses. Therefore, you can submit the following formto become a member of the Honors Program in the event that you do not get chosen for the Scholars Program.
