The YSU Student Health Clinic is offering H1N1 vaccinations 8am-3pm daily [M-F] to the YSU community.
 
You must be 18 years of age or older to receive the vaccine.  The vaccination is FREE.  However, you must bring with you a completed online H1N1 Registration Form to be validated to receive the vaccine.  [See details below.]  Family members who are 18 years of age or older may also receive a vaccination if accompanied by a student, faculty, or staff member with a current YSU ID and have their completed online H1N1 Registration Form.

The vaccine will be administered on a first-come, first-served basis while supplies last.  No appointments will be given.  The Student Health Clinic is located on the first floor of Kilcawley Residence Hall, adjacent to Kilcawley Center.  The entrance faces University Plaza.
 
IMPORTANT YOU MUST DO THIS:  You must present your completed H1N1 online Registration Form to receive the vaccination at the YSU Student Health Clinic.  The registration process requires a printer.  The form is simple and available at https://h1n1vaccine.odh.ohio.gov .  To get started, find the ‘Individual Login’ box located on the right side of the page.  Click on ‘New Individual and Family Pre-Register’.  The form will ask if you are the ‘Head of Household’.  Please mark this box YES so that your form will process.   [The H1N1 Registration form cannot be completed at the Clinic.  If you do not have your form, you will need visit a campus computer lab to complete and print out your form.]
